TUSB7340: USB device is not detected

Our customer has trouble with mass-production in the market.

When USB mouse (low speed) is connected to USB_DP/DM port 4, it can not be detected.

The other ports (1 - 3) can work with no problem.

I attached USB_DP/DN waveform.

As you see in waveform, USB_DP_DN4 rises to ~1.5V accompanying with DM_DN4.
As a result, bus reset is not done and USB mouse is not detected.

Could it be suspected the device is defected? (e.g. EOS)
Or could it be caused how to use the device?
