2, How to set up Address of PHY? But I can't find introduction of PHYAD .

For Phy-Address: you have to use Straps ( Pullups/PullDown) on RX_DV and RX_ER. Refer to Table 19 in datasheet. For schematics, review, kindly use attached xls to review the pin connections.
