Does this happen repeatably across several chips?
This sort of damage is usually the result of of exposing the IC to voltages outside of its absolute maximum supported ranges. to debug, you may want to probe various pins like VCC, A, and B during power up and throughout usage to verify there are no unintended voltage spikes.
