TPS65981: Issue with blown dual MOSFET and also negotiating up to 15V in some cases

Hello,

We have a design that tries to act as a power delivery source for charging while at the same time acting as a upward-facing port for the data role. This has worked for us but on a few units we see the external dual MOSFETs being destroyed and also issues while negotiating up to the 15V contract - it seems to trip something inside the TPS while trying. 

Any chance somebody more skilled than us can take a look at the attached json config file for a quick sanity check?

    I do not see anything attached. In general though, the external FET being damaged is likely due to the system operating outside of the SOA curve for that FET for a period of time. 

    Is there a specific test you are running where you notice the FETs beginning to fail? For example, what are you having act as your PD sink device? If a large amount of current is being drawn while VBUS is still ramping, that voltage differential across the FETs coupled with the sink current could cause an SOA failure.
