CD4081B: damage issue

Hi Ti experts,

My customer is using a chip: CD4081BM96 SOP14. The damage rate of this chip is very high. It often breaks down after a period of use. Is there something wrong with our wiring circuit?
The 15V voltage is generated by a 7815 three-terminal voltage regulator tube and directly connected to the 14-pin VDD. The maximum input of other pins will not exceed 15V.

I am currently testing a broken chip, and its pins 2, 4, 5, 6, 8, 9, and 10 are all short-circuited to the ground.

I am think if it is caused by input pull-up before chip power supply due to C71. Or R94's value is not enough to limit the input current.
