i have attached a pdf of the schematic. just to reiterate, the TI devices work great. we are doing something wrong on our 1 module. we use 100s of these components all over our system and chassis and they work great. I also have a part that we can send you to analyze.

thanks for the schematic . Noticed that the part you mention is SN74CB3Q3245PW in the post but the schematic has SN74CB3T3245PW . Noticed that there are A8, B8 ports left floating which is not recommended . Can you tell me what are the voltages from FPGA going into this part . Is it only Module 1 which is blowing up ?We will look at your schematic and reply back soon.

we are using the SN74CB3Q3245PW. FPGA Interface is set for 3.3V I/O. It is the same module where the FET is blown. This Module is handled quite a bit by users. It is plugged in/out of the Chassis many times. The other modules are plugged into the Chassis and remain there.
