TM4C1230C3PM: Sub-licensing of VID, unique PID for Education

Hi Team,

For the TM4C12x series microcontrollers TI had published a USB VID:PID Application (https://www.ti.com/lit/ml/spml001/spml001.pdf) for sub-licensing TI's VID and providing a unique PID. The application note has a link saying to fill out an online form (http://software-dl-1.ti.com/dsps/forms/vidtrackerMature.html) but the link is not valid.

Is there an updated link, or alternative solution for educational bodies to sub-license TI's USB VID to create a unique PID?
