What do you mean by a "random way?" Are multiple boards failing? Are the board intermittently failing?
I usually run into this kind of issue when there is a physical connection issue.

Yes, several boards have presented problems. 3/4 out of 35 boards gave error when the students try to debug/download the code. In these cases, we solved the problem following instructions from XDS110 Support Notes document. I guess students cancelled or unplugged the board while updating firmware and the boards remained in DFU mode.
But there is one board that gives this error (Error -615 @ 0x0) that I don't know how to solve. Also, I don't know exactly when the board stopped working. It is possible that it stopped working past year and somebody store in the cabinet, or the problem appeared past week.
I know that it would be very convenient to have information on how and when the problem happened, but we don't have this information.
I agree with your sentence "I usually run into this kind of issue when there is a physical connection issue." But I have no idea where start to look for a broke soldering or pin.

Hi Toni,
You could try using one of the good boards to program the target on the bad board --> This would tell you if the XDS is malfunctioning.
This is done by wiring the XDS jumpers from the good board to the bad board.
