The issue looks like a configuration issue. 

Can you please run the qspi flash transfer example and see if the example is passing in 4s-4s-4s mode in your custom board? This will be a valuable test to conduct. If this test passes, it gives us confidence in the configuration.

OK, that's not in v11 SDK. I infer that it is only in the v26.x.

Can I use v26.x with Eclipse?

You mean Code Composer Studio 12.8.1? 
Yes , SDK 26 can be used with CCS Eclipse versions as well

There is no qspi folder.

Since it is AM263P, the example is ospi_flash_io. Could you please run this example on your custom board with 4s-4s-4s protocol.
