I have a TMS570LS12CNCD control card. The user manual says there is a BOM available:

TMS570LS12CtrlCard_BOM_RevB in Excel format.

I can't find this anywhere on the Motor kit DVD or what gets installed from DVD. Where can I get this file?

The Excel file is in the zipped archive on the same page that you referenced, the fourth link under the "Control Card Schematic" section.
