My customer report an issue that 570 was destroy when their product are running, they found over ten case about this issue, and send one to us to do FA, accroding to the FA report, Pin 2，3，4 and VCCIO are shorted to VSS, in fact the Pin 2, 3, 4 are not used and there is no connection on these pin, the VCCIO was connected to TPS65381 3.3V output(VDD3/5)，and TPS65381 circuit was design refer to our reference design, If they remove the burned 570 and replace a new one, then everything return to work well.
the issue occur in their end-customer, and customer did a lot of test in the lab but has not see this issue occur, do we have any suggestions to help customer do analyss to find why 570 was burned.
