Can you please try again. I'm able to download it fine. After you submit the form you should get an email from TI confirming the approval. If you have problems with your download, please contact ti_techdoc_exportapproval@list.ti.com
Actually I get a 404 error, after I press submit on the page you just made a screenshot, and no e-mail in my inbox.

If you submit and have been approved then you will see this page. Can you try a different browser? If you still don't see the below page and neither the email then perhaps there is some issue with the approval process. Please contact ti_techdoc_exportapproval@list.ti.com.
