As you can see, there is a wide range of potential failures. The actual failure will depend on the device, system ESD protection, where the ESD is applied, and the level of ESD.

This is too general a question, pretty much anything can happen, from nothing to a single shorted output to a complete latchup of the chip that may or may not be recoverable be removing and re-applying power. In some cases, the watchdog timer can be used to reset the device if it is not "fed", reset periodically.
