DRV8306: DRV IC failure

We are using the IC DRV8306 & the boost LM5121. Boost section we loaded upto 10A with electronic load. Also DRV we run upto 10A with the motor. Both are working independently. We had tried many times, no failure observed. But soon after integrating both boost output & DRV input , the DRV IC failed at loading 2A. The second unit IC failed in 3A. In DRV IC 3.3V generated from DVDD pin. In failed boards this pin is found to be short to ground. Given below the screenshots of both the schematics and the damaged IC. In damaged IC a black spot is observed. What may be the possible reasons

The board was deisgned by us. Two boards one contains boost & the other has drv. Both are connected with wires

I tried to taking a look at the schematic you have uploaded; however, the picture is quite blurry when I have to zoom in. Could you please upload a pdf of the schematic so that I can look at the values of the components used.

Thank you for your patience.

In the attached PDF you will find some of our concerns and recommendations in the comments.

The damage on the board could be due to the ESD diode becoming reverse biased leading to physical damage.

It might happen if difference between VM and VCP exceeds 15V. Potential reasons include:

large amount of current at SHx
VM voltage sinking faster than VCP due to the presence of a larger (2.2 uF vs 1 uF) capacitor at VCP
Not enough bulk capacitance between VM which could lead to VM dropping quickly.

In order to understand what happened please monitor the waveform of VM, VCP and SHx to check if they are in that range.

Furthermore, please check if the DVDD capacitor is blown to understand if the short is through the device, or through the capacitor by de-soldering the IC and testing. It is possible that the short could be through the PCB itself.
