DRV8353R: DRV8353R Charge pump fail permanently. What might be the problem?

I am recently using the DRV8353RH into my design, for BLDC motor control application with 60V power supply.

And I face an issue : 

1) 60V motor startup NG (couldn't recover), the IC CPL pin (Pin number 3) have obvious low impedance(60ohm), normally should be at least 100+Kohm. (IC damage but not blowup)

2) After changing IDRIVE setting from 450mA/900mA to 150mA/300mA and add a high value bulk capacitor (600uF) --> Motor startup to free run Pass.

3) With the setting above, and we add load to the motor then the whole DRV8353RH blowup. (the MOSFET is fine and very clean, very sure the heat is not from the MOSFET)

The blow up IC has very obvious burned damage at IC's Pin1~Pin7 which I guess is the charge pump module. (the IC have smoke occur)

The PCB PAD beneath the IC is heavily burned, the copper dislocates.

I have this test fail twice, which I am very sure isn't a coincidence. Both times the IC's Pin1~Pin7 have obvious damage and the MOSFET are both time fine. What might be the problem?
