We are experiencing failures of the TPS2378 when on auxiliary power. Our auxiliary power is a 48V AC-DC power supply. This is only happening with a handful of devices. I've attached our schematic and pictures of the damage. Please advise on potential root causes, and let me know what other information is needed.
