TPS54062: High input current failure

I have a product that uses the TPS540062. 

I am experiencing similar issues to the

I have around 5% of boards failing with a high current on the input which blows the chip with smoke appearing if there is no current limit. We also have a smaller number of units which take no current on the input.

In both cases replacing the TPS540062 fixes the issue.

All products were working for a while before they failed, and more are failing over time.

The units were built in 2015-2016.

I was wondering if you have any information relating to resolving the above thread.
