TPS2456A: MOSFET Q6 in our hot swap circuit will be damaged (become short between drain and source ) on some boards

We find the MOSFET Q6 in our hot swap circuit which uses TPS2456 solution will be damaged (become short between drain and source ) on some of our boards.  The schematics, MOSFET datasheet, fuse datasheet are attached.  Could you help to review the schematics and give your comments about the failure? 

And we have done such a testing, deliberately make +12V short to DGND, we found the hot swap circuit can't protect the MOSFET or the fuse, we found both of the MOSFET Q6 and the fuse F1 are broken after the short circuit (only +12VA is present in this testing, +12VB is not present).  Why?
