Are you sure the absolute maximum rating of the VCC pin is not getting violated? The absolute maximum voltage on the VCC pin is 20V. What is the voltage rating for D6? The new UCC28C53 has a 30V maximum VCC rating it might be more robust if this is the issue. This is a pin-to-pin direct drop-in replacement.

D5 in your schematics seems not typical. Does this design have high negative spikes, or why did you add D5 in this design?
Can you capture the OUT pin waveform when turn off?
