BQ34Z100-R2: BQ34Z100 shows a steady and accurate SOC while charging up to 100% but immediately drops to 0% after reaching full charge

We are working on a custom PCB with BQ76952 and BQ34Z100-R2 fuel gauge. We're using a 12 cell battery with 50.4V total pack voltage. The battery pack is custom made so we required to determine a CHEM ID. 

I have already done the following steps:
1. Used the GPCCHEM tool and generated a CHEM ID (Here's my report )

2. Performed a learning cycle and update status even changed to 0x06.

After performing the learning cycle, I charge the battery to full and the SOC shows accurate values upon charging. However, as soon as it reaches full charge, the SOC abruptly drops down to 0% and remains like that until I discharge it. Then in the next attempt, the SOC at charging is valid again. Please help, as I could not find any solution to this.
