My resolution:
I just had the same issue with startup of the LT1054, my load was an opamp and analog mux. < 55mA load
I had to wait > 30 seconds for a power cycle and even then, the LT1054 would latch at ~0.8V sometimes.
The solution is two parts:
1. The LT1054 needs the NPN transistor that is in the data sheet with a light resistive load to the positive rail, tune the base resistor to achieve good Vce saturation.
2. I also had to reduce the - output cap from 100uF to 10uF ceramic. I put the 100uF after the - output of the NPN transistor by the opamp for low ripple.
The end result was I could power cycle << 1 second with 100% startup.
