You are dissipating ((12V-5V) * 250mA) 1.75W in the device.
The DCY package (SOT-223) has a stated junction-to-ambient thermal resistance of 104.3°C/W.
Internal Temperature = Tj = ((1.75W x 104°C/W) +Ta) = (182°C + 25°C) = 207°C
Unlikely that you will ever get to 207°C as internal thermal limiting should kick in at a much lower temperature.
The KTT (TO-263) package would be much better choice for this power dissipation level
Internal Temperature = Tj = ((1.75W x 27.5°C/W) +Ta) = (48.1°C + 25°C) = 73.1°C
It will still feel hot to the touch, but at least the package will be able to handle it.
