    There are some parts of the schematic that I'm not sure what you are trying to accomplish. For example what is the reason for Q8?Q7 is for start-up are to you trying to turn off the start-up FET?  I'm also concern that the VBE of Q8 will affect the UVOL turn-on over temperature?  Your feedback compensation R40, C11, and C13 are connected to the LM5022 COMP pin, but the other end is open.  I would not connect the OPTO and compensation the way you have it hooked up.  A simpler way to to connect the emmiter of U4 to ground, then connect the collector of U4 to the COMP pin of the LM5022.  Then ground the FB of the LM5022.  You have a loop compensation network on the secondary R46 and C3.
