Hello JH,

1. Yes. 

2. No. the signal is determined at the auxiliary power input J3.

3. The datasheet for the part is unclear. Q1 and Q4 are integrated MOSFET input rectifiers. You can chose to use MOSFETs or diodes, and you can choose discretes  or something integrated. Since you are using Type 4 power, I would suggest using an integrated MOSFET rectifier, like in the EVM, because the power loss is going to be more apparent versus a Type 1 device. Otherwise you can loose 2-3% efficiency on the input bridge alone. 

4. You can find reference designs online by using a search engine, and typing in either TI PoE reference designs or TI DC_DC Reference designs.
