AM62A7: Custom board unable to connect through USB or UART

Hey TI team,

We have designed a custom board with AM62A7 using the SK as a template.  The board has the same DDR as the SK. 

The boot modes are selected as eMMC for primary and USB as secondary show here: 1101001100100100 (B0...B15)

The board uses discrete converters for the power sequencing handled by LM3880.  Here is the detailed info of the sequencing:

16 ms interval between each flag to allow for AM62 clock startup
Startup Sequence 1-2-3-4-5-6
Shutdown Sequence 6-5-4-3-2-1

Voltage for each flag:
1. 1.2V (camera)
2. 1.8V
3. 1.1V (LPDDR)
4. 0.85V (Core)
5. 2.9V (Camera)
6. MCU_PORz

Upon power up the converters are outputting the designed voltage, the clock is running at 25 MHz, MCU_PORz is high, RESETSTATz is high, and PORzOUT is high.

Unfortunately, we are unable to establish communications over USB.  We have used both a Linux and Windows machine and neither recognize the USB port.  UART is also silent.

It seems something is missing here and your advice would be much appreciated.
