EVM6678L, problems with XDS560v2-USB mezzanine card

Hi all, 

I have a dev-board TMDXEVM6678L with XDS560v2-USB mezzanine card atached, and I have a problem with it(xds560).

RED-GREEN-YELLOW-LEDs was in flashing state, and I tried to clear this state, as described in http://processors.wiki.ti.com/index.php/XDS560v2_System_Trace.
Now I don't see any USB device  in windows device manager, and the LEDs off the xds560 mezzanine card are as in the atached picture.


What is the problem? and how to resolve it.
