UBIFS failures

We designed a custom board that has a DM8168 chip using a Micron MT29F4G16ABADAH4-IT:D NAND flash for our filesystem.  I went through the instructions here: http://processors.wiki.ti.com/index.php/UBIFS_Support to build my ubi.img file and to mount it with ubiformat.  

My ubi.img is built with the following commands and configuration:

The ubi.img is mounted with the following command:

This works fine for some time and across several power cycles.  But after 1-30 days, we start seeing UBIFS errors.  An terminal capture is attached that gives an example of one of the failures.  As I capture more failures, I will attach additional captures.  
I have tried making the filesystem readonly in the bootargs, but that didn't seem to fix the problem either.  
Any recommendations?  I searched UBIFS in this forum and had so many hits, I didn't even know where to start.
