There are multiple ways of approaching this to create combined ARM +DSP application image. You haven`t mentioned what software and device you are using, let me describe some of the options :

If you don`t have a secondary bootloader but want to do this directly using AISGen tools then refer to the wiki here:

If you want to use a secondary bootloader then refer to the following options::

The serial flash and boot utilities package provided on the link also provides a CCS based NAND, NOR and SDMMC writer that can be used for flashing. Please download the Serial flash and boot utilities package and look under :
OMAP-L138_FlashAndBootUtils_2_40\OMAP-L138\CCS directory path
