Hi Naveen,
Firstly, please take into account that the EVM is not a reference design and should not be copied blindly. I'm saying that because of so many issues in the past coming from the directly re-used parts from the EVM.
Secondly, please be aware that I'm not directly involved with EVMs or schematic designs but what I know is:
- Rev E is with SoC revision 1.0. It is highly recommended to use SR2.0 and EVM rev H. 
- for the reset issue, use one the latest PMICs. They can be instructed assert a POR signal upon warm reset, thus eliminating the warm-reset issue. See the PMIC BOOT pin setting for your PMIC so it gets instructed to assert POR on warm resets. See your PMIC datasheet.
