Please see the C6457 datasheet sprs582 page 56 for part number dissection. Also, please see the C6457 errata sprz293 page 6 for additional details on the package markings.

Thanks for help, but I have not answer yet. Typically TI describe silicon rev at package as wrote in errata, but
$414-09A4SHW is not compatible with template "#xx-#######". Is symbol '$' just point to new silicon manufacturer, then what about "414"?

The xx field is the 14 in your package markings. It is odd that this conflicts with the part number which should have an extra C between the 6457 and CMH.
From the datasheet, you can tell the speed grade from the package marking. No speed suffix means 1GHz speed grade.
Can you read the revision registers that are described in the errata document? This would settle the question on silicon version. What is the source of the device, EVM, prototype board, customer board, or such?

looks like truth - despite absence 'C' before "CMH" in package marking this DSP presents to me as CCMH.
