Mark, Pins 1 & 5 are missing from the Spice and TINA models because they do not support the Balance feature of the actual device. Spice models cannot track all of the variations of a real device, like random part-to-part variations in output offset, bias current, temp drift and so on. Because of this, the balance feature was left out of the model.
