Microphone Pre-amplifier

I am looking for a bit of information to get me in the right direction with a microphone pre-amplifier, and how to utilize AC current for power in a professional grade mic-pre. I should let it be known that I'm aiming for a high quality 1 channel input, with +48V phantom power, with a balanced output, imam using an INA127 Instrumentation amplifier, an Op-Amp, and a Balanced line driver. What is the easiest way to incorporate AC power that is clean? How can I make it as noise-less as possible? It's analog.
