LMZ21701: No 3.3V voltage regulation

Part Number: LMZ21701

My application needs to step down 12V to 3.3V. I used LMZ21701 for its compact footprint. My schematic:

But output voltage is not 3.3V, instead it appears to track input voltage, Vout = Vin - 2.1V. At 12V input, I see 9.9V at Vout. How do I achieve correct voltage regulation?

    Either the module is damaged or the FB resistors are not soldered correctly.  Please check them.
