LM5116 Output Voltage drops to 3V when Input drops to 3V

We are using the LM5116 to step down the input voltage from 12V to 100V input to 12V. But when we power the LM5116 with an input voltage below 17V it outputs 3 Volts and if the output goes above 20V then the output voltage is unstable.

    Please make it sure that your VIN pin voltage is stable. You need a Cvin capacitor close to the IC.
    Please check whether your UVLO pin voltage is stable. Any hiccup operation?
    please check your VCC is up.
    It seems your C29 and C30 should be swapped each other.
