UCD90120A OC and Rail #1 Error

Hello - I have a Xilinx ZC706 board with UCD90120A sequencer. All of a sudden, my board isnt working. I did some debugging and found that a few of my Voltage enables were not being set high by the sequencer. I purchased the TI USB to GPIO adapter and followed the instructions for reprogramming the UCD90120A using the ZC706 xml and the TI Digital Power Manufacturing Tool. I run through the script to load the xml settings and everything gives me a green checkmark until I get to the Validate_101 step, where I get a "READ_OUT on rail#1(0.001V) was not within 5% of VOUT_COMMAND(1.000V)" error. When I power on the board, I can check that poins 1,2, and 8 are shorted to ground and not at their appropriate voltage levels.

How can I resolve this issue?
