As you can see from figure 7, the AGC doesn't activate until 150uA. Have you considered the recommended alternatives ONET8541T and ONET8551T? They are newer parts.

You might want to take a look at the attached file for the ONET8551T EVM design. The ONET8551T used the same EVM as the ONET8541T. The two additional pins 17 and 20 on the ONET8551T can be grounded for additional bandwidth.

The ONET4291TA only has around 3GHz bandwidth. If you need higher then I suugest the ONET8541T.
If you bias the PD cathode externally, the AGC will not function in the 4291TA.
There is no way to disable the DC offset cancellation.
