Is it necessary to be FET input?
If not, the THS4012 meets the above criteria. It is unity gain stable with a BW of 290 MHz, 310 V/us slew rate, 7.5 nV/rtHz noise, and comes in a dual SOIC package.
Based on your needs, there are two recommendations off the top of my head:
a) THS4032 - lowest noise
b) THS4012 - more speed and bandwidth at the expense of a bit more noise than the THS4032, but is <10nV/rtHz
