OPA455: Unipolar Mode

Hi Team, assisting a customer for their application. Seeking some clarification. 

As per the datasheet we can operate the from 12V to 150V and +/- 6 to +/-75V. We are operating the device inD and applying 78V and the negative pin is connected to ground
Could you please help us what can be the wrong. In our logic we are using the circuit as simple adder to add two voltages and apply a gain. We are observing the failure at the supply pin.

    If properly connected and applied the OPA455 is usable in a single-supply application. Please send us your OPA455 circuit diagram with the supply voltage, input voltage range and source and output load information. We can look it over and see what it is you are doing with the Op amp.

    Since you are using a single supply configuration I assume the OPA455 underside PowerPad is being connected to GND?

    With a single supply rail, you need to watch for the input Vcm and Vout swing range. Vout swing range is a function of load. Please make sure that the circuit is not exceeding the required output load current. which it is able to drive approx. 45mA. 

    Please share the circuit in questions with us. I put an example below in a single supply rail with Gain = 6 V./V, but this is just an example.
