LMC6042: Sample layout with guard ring for SMD device

Hi Team,

Our customer is using the through hole version of LMC6042. They want to decrease the board size so they will use the SMD version. He would like to know how to implement the guard ring in the SMD version. Figure 33 to 36 of the datasheet shows the electrical connections based on the configuration but didn't show sample layout for SMD device. Do we have sample layout for LMC6042?
