THS4551: output is came but both are in phase only

Do you have your common mode control pin at the power supply? 

Might just float it and see if things work better, it will default to mid supply floated.

the maximum supply voltage of THS4551 is 5.5V. You run it at +/-5V = 10V !

maybe we have a language problem?

Your supply voltage is wrong!

Take +/-2.5V or +5V/0V but not +/-5V.

PD 7th pin should  be GND OR VCC?

your questions are all answered in the datasheet. We spent a lot of time putting that together to help designers achieve correct performance - you should perhaps peruse it a bit. 

PD needs to be at the +Vcc pin if not used.
