will be hard to find a 100% replacement. I would look at the TI's ADLS / DSL drivers:

Kai is correct, this very early high power out CFA by LTC was never really suitable to DSL and nobody tried to match it - All the real DSL drivers of that time (and now) are dual push pull devices - the TI THS6xxx series. That TI group also re-released some of these as dual and single THS3xxx +/-15V devices on BiCom1 process. There might be something there, but nothing in that LT1210 package nor 1A output spec.

In addition to our THS6XXX and THS3XXX families of CFAs, the OPA2673 could be a reasonable replacement option. Without more information, I cannot be sure which specifications or parameters of the LT1210 are valuable to the customer.
Please let me know if members of either family, or the OPA2673, would be a good fit for your customer's design needs.
