I couldn't find in TI portfolio high voltage high performance stand-alone opamp which is similar to output opamp A3 inside INA103. Does it exist at all? Thank you

There is no stand-alone op amp that is exactly the same as the output op amp of the INA103. There are no specifications for the output op amp alone so I'm not sure exactly what you are looking for. From my memory, here are the approximate specifications of the output amplifier:
GBW: 10MHz
Slew Rate: 15V/us
Noise: 8 nV/rt-Hz
Please check out the OPA140 op amp family as this would be pretty close. If you want an audio version of the same general design, look at the OPA1642 and OPA1644.
If you are looking for an op amp with very low voltage noise, like the INA103, check out OPA211 (or OPA1611 audio version). These op amps are much lower noise than the output amplifier of the INA103 but more closely match the input noise of the INA103.

I realized that you may be looking for an op amp that runs on power supplies greater than +/-18V. Look at the OPA604 (single) and OPA2604 (dual).

While INA103 is an old product, I'm still considering it as the best mike preamp chip because of (but not limited to) +/-25V output swing. I couldn't find matching part which can serve as the output/driver stage for INA103 based mike preamp. OPA604 is noisy as 7k resistor. I'll give it a try with external input stage.
