LM358B: Amplify signal from "0-2.5V" to "-15 to +15V"

See Designing Gain and Offset in Thirty Seconds. The mentioned tool is now available at earmark.net/gesr/opamp/.

If you have only ±15 V supplies, then you need an opamp with rail-to-rail output, e.g., TLV9301. (But no rail-to-rail output is perfect, especially with a non-zero load. Consider wider supplies.)

You will need a reference voltage; supply voltage an be used if it is accurate. 

LM358 supply voltage would need to be at least +17V and -16V but difference supply must be less than 36V.
