Hi -
Customer is reporting an issue where they are powering the op-amp before their signal gets to the input of the op amp. Not sure the state it is in prior to signal present, might be floating? I don't have schematic and I'm also not sure the configuration of how they are using it.
They said they were seeing permanent damage on the op-amp and had to add a diode to protect it from this scenario.
Question for you is - have you seen this type of failure before? Any watch-outs you have with regards to avoiding this failure mode?
