INA185: Please review the schematic

Dear TI experts,

My customer have a problem with operating INA185A1IDRLR. Could you review the schematic first?

The problem is, sometimes There is no output on pin 1. And This is not related to the part.

SO we suspect that some conditions are exceeded the specification of INA185A1. Could you also check the operation conditions below?

- idle condition : IN+ and IN- are applied about 4.7V.

- operation condition : output is about 0.15V when IN+ is 0.133V and IN- is 0.057V.

Please check this issue, and let me know if you need more information.

    It's possible that device was damaged during some transitory system voltage surge.

    1. How many systems/devices are showing this behavior?

    2. Does a abnormal device ever regain expected functionality and accuracy?

    3. For an abnormal device, can you measure input bias current (via the voltage drops across R8 and R67) during all conditions?

    4. For an abnormal device, can you try measuring supply current into VS pin?

    5. Are there any voltage surges possible in you system operation? Is the board being hot-swapped (plugged into and out of voltage sources without powering down first)?

    6. Can you swap the INA185 devices from a working board and an abnormal board (A-B-A) swap to see if the problem follows the device?

    I got an update from my customer about the issue above. It is because of the cold solder.
