Why the Vcm is from -0.2V not 0V?

The reason that the INA181 can tolerate negative inputs is we have ESD protection diodes on the input pins that allow the device to tolerate negative transients. This way, it makes the INA181 more robust when used in a system.
I want to also point you to the ABS max table where it is noted that the abs max negative input is GND - 0.3V.
We recommend at most to have the inputs 200mV below the ground for the device and we warranty this performance.
The abs max table tells you that the device will be damaged if the inputs go below GND - 0.3V.
