We had in the last 2 years 10 EOS/EIPD cases and want to find out the root cause. Can we look together in details and define teh next steps?

    I’ve reviewed some of the summaries and reports. It appears TI QRE has suggested certain investigations, such as capturing pin voltages including inputs and supply. Has this been done?

    I assume the failure rate is low, therefore it is important to report as accurately as possible the conditions under which devices failed. Because voltage spikes may happen under certain conditions only.
