OPA817: Slew Rate not keeping up with input

I am using an OPA 817 in the voltage follower configuration with a o 0 ohm in the feedback path (connecting pin 2 to pin 3).  I have a 76.8 MHz 1Vpp square wave on the non-inverting input and a 100 ohm load.  The resulting output waveform is showing a 350 mVpp sawtooth.

I have tried removing the load on the output R53_0 and still same behavior.  It looks like there are two slew rates, but the signal is slewing about 350 mV in 6.5 nSec which is about 50 V/usec, when the part is specified at 1000V/uSec.  Would love to hear your thoughts on why this behavior is not matching data sheet specifications.

    Just to ensure the device is not damaged, could you please confirm the Iq of the device when not driving the device. Is it around the typical value specified in the datasheet? Do you see the same effect if you slow down the input signal or is this input signal frequency fixed?
