LMC6482: Circuit output inconsistent with theory, unity-gain inverting circuit

My customer use LMC6482 as unity-gain inverting circuit .

At present, it has been found that the input and output are 105mV different, 0.615V input, 0.720V output. 

Do you have some suggestion?

    Could you provide more information about the circuit? Some information that would be helpful include power supply voltages, passive component tolerances, and the frequency of the input signal.

    Also, additional input and output voltage measurements would be helpful for characterizing the error. For example, with a 1V DC input, you should expect a -1V output.
