I'd guess that you are trying to output a value below the negative supply (V−), which is not possible.

Printed board as long as it is higher than 300mV or so will be abnormal, in 100 to 200mv, the waveform is normal

In theory, ±2 V should be possible with these supplies.

Please check the supply voltages, and show a trace of all three I/O pins of U7.
